CRICO’s Underwriting department provides confirmation of your coverage and claim histories for physicians and employees—currently or formerly insured by CRICO. There is a small fee for claim history requests for those who are no longer insured.
You will be prompted to determine which form will appear to help you receive your information in the most efficient manner possible.

While currently insured MDs can use the physician portal page to download their latest confirmation of insurance documents, there is also a form for all non or ex-MDs to make a request without logging into the portal.
